Advance Care Planning

Advance Care Planning is a process which helps a person plan their medical care in advance so their wishes are respected by healthcare teams, families and carers when they become too unwell to make decisions for themselves.

Advance Care Planning Working Group

The Advance Care Planning (ACP) working group of the Ageing Well Alliance aims to enhance end of life care through the development of consistent ACP resources and programs for consumers and service providers. Member organisations of the working group include:

  • Commonwealth Respite and Carelink Centre
  • Frankston City Council
  • Frankston-Mornington Peninsula Medicare Local
  • Peninsula Health
  • Peninsula Home Hospice
  • Mornington Peninsula Shire
  • Royal District Nursing Service
  • SomerCare Residential Aged Care Facility

The ACP working group has developed a range of resources to facilitate the consistent use of ACP in the Frankston and Mornington Peninsula catchment.

These resources have been developed for the use of organisations and individuals in the Frankston and Mornington Peninsula catchment. Organisations outside of the catchment should contact the ACP service for free permission to use the documents. Contact:
