Advance Care Planning
Advance Care Planning is a process which helps a person plan their medical care in advance so their wishes are respected by healthcare teams, families and carers when they become too unwell to make decisions for themselves.
Advance Care Planning Working Group
The Advance Care Planning (ACP) working group of the Ageing Well Alliance aims to enhance end of life care through the development of consistent ACP resources and programs for consumers and service providers. Member organisations of the working group include:

- Peninsula Home Hospice
- Mornington Peninsula Shire
- Royal District Nursing Service
- SomerCare Residential Aged Care Facility
The ACP working group has developed a range of resources to facilitate the consistent use of ACP in the Frankston and Mornington Peninsula catchment.
These resources have been developed for the use of organisations and individuals in the Frankston and Mornington Peninsula catchment. Organisations outside of the catchment should contact the ACP service for free permission to use the documents. Contact: firstname.lastname@example.org
Advance Care Planning Forms
A free Advance Care Planning Toolkit is available online to guide you to create an individualised form for personal use. At completion you can email it or save it to your computer. Click on the link below to access the online form required.

Peninsula Health provides free assistance with Advance Care Planning for people living in the Mornington Peninsula and Frankston City council regions. Detailed information and guidance to complete the necessary documents is available by appointment or phone advice. Ask your doctor, nurse or health care provider to make a referral for you or contact the Advance Care Planning Service directly. If you live in an aged care home or health care facility ask your nurse or GP for assistance.
